STEP 1:  FILL OUT RESERVATION FORM IN MRS. MARSH'S OFFICE. 
              NO reservation forms will be accepted after June 11, 2009.

• Students with outstanding textbooks, fines, or library materials from the current year will NOT receive summer texts.
• Only 2 books per student allowed.
• Student must be enrolled in the course next school year to receive a book for that course.
• Not all classes have textbooks available for check out.
• Books are only available for current Lynbrook students. Incoming students are not able to receive textbooks until the start of school.
• A reservation form be completed and signed by both the student and a parent or guardian and returned to Mrs. Marsh no later than the end of school on June 11, 2009
• Fill in the Textbook Title and the Course name in the chart below for EACH book you wish to check out.
• Keep the YELLOW copy. You will need to bring it with you to the library to get your book(s).

STEP 2:  PICK UP YOUR TEXTBOOK(S)

When:   Check out - June 15-19    9:00 – 11:00 am 
Where:  Library 
What do you need:

• Student ID card required for check out. No card - no book.
• All delinquencies and books must be cleared to receive books for summer study.
• Signed copy of reservation form. (yellow copy) NO FORM. NO BOOK.

STEP 3:  RETURN OR RENEW TEXTBOOK(S)

• MONDAY or TUESDAY  August 24 25, 2009  

• Students will not be allowed to check out textbooks when school opens unless all summer texts are returned or renewed.
